All referrals are reviewed by the management team, and a pre-admission assessment is carried out to ensure that the service can safely and appropriately meet the individual’s care needs. This assessment may involve reviewing medical history, current care requirements, risk assessments,and discussions with the individual, their family and relevant healthcare professionals.
Following the assessment, if Parklands is able to meet the person’s needs and a placement is available, arrangements will be made to support a smooth admission into the home.
