Great Minds Together

Great Minds Together provides neuro‑affirming support for children, young people and families with special educational needs and disabilities (SEND) or social, emotional and mental health (SEMH) needs. The service offers integrated outreach, therapeutic intervention and family support to improve stability, wellbeing and access to education and community life.

Support is tailored to each child or young person through individually designed intervention plans. These plans are reviewed regularly and delivered by a multidisciplinary team using a trauma‑informed and relationship‑based approach. The service works across education, health, social care and housing to help families navigate complex systems and reduce the risk of crisis or escalation.

Key programmes include the 5th Avenue Project, which provides outreach support for families experiencing difficulties accessing SEND, mental health or education services. It also offers a food and school uniform bank for the community.
The Thriving Futures programme (Tier 3.5) offers intensive therapeutic support for young people at risk of hospital admission, exclusion, placement breakdown or police involvement, including step‑down support from Tier 4. Living Well Spaces provide safe, supportive transition environments for young people who need stability and structured therapeutic input.

Great Minds Together accepts referrals from families, schools, health professionals, social care teams and partner agencies. Some services are free to access, while others require commissioning or agreement with local authorities or integrated care boards. Support is available across Greater Manchester, Lancashire and surrounding areas, with some programmes delivered nationally depending on commissioning arrangements.
