Charges apply depending on the type and level of care required. Costs are agreed following an individual care assessment. Referral Details
Referrals can be made by individuals, family members, social workers, NHS professionals or other healthcare providers. Self-referrals are also welcome. All referrals are followed by an initial assessment to ensure that the service meets the individual’s care and support needs. -
