Community Care Team

Since 1992, Community Care Team has been delivering high-quality home care services. We cover Preston, Longridge, Chorley, South Ribble, and surrounding areas including Leyland, Penwortham, Walton-le-Dale, Lostock Hall, Bamber Bridge, Euxton, Whittle-le-Woods, Clayton-le-Woods, Astley Village, and Buckshaw Village.

Our mission is to enhance the quality of life of individuals who wish to remain in their own homes while receiving the support they need due to illness, disability, or age-related frailty. We also provide respite care, offering valuable support to family members and carers. Services are tailored to individual requirements and can be funded through Social Services, personal budgets, direct payments, respite hours, or private arrangements.

We are proud of our dedicated and experienced team, who are committed to delivering compassionate, person-centred care. All staff receive appropriate training, hold relevant qualifications, and have the experience needed to provide professional support while respecting the comfort and routine of each person's home environment.

Our aim is to promote independence, dignity, and wellbeing through the provision of personal care, social support, and practical assistance. We achieve this by supporting people to maintain their chosen lifestyle, working collaboratively with families, carers, and healthcare professionals, and delivering services in line with established policies, procedures, and best practice standards.
